New Patient Health check
As a newly registered patient we would like you to provide us with a height, weight and blood pressure reading. If you do not have a blood pressure machine at home you can use our blood pressure machine which we have on our reception desk.

When we are closed
GP surgeries provide NHS medical cover 24/7 and when our surgery is closed we ask you to call 111, free from any landline and most mobiles. A call handler will assess your needs and advise you what to do. In a genuine emergency you should call 999. Chest pains and / or shortness of breath constitute an emergency.
Appointments
Appointments can be made in person or by telephoning 01252 229840 during surgery hours. To book an appointment with a GP, you will be offered either an urgent appointment which are only bookable on the day or a routine appointment which will be in advance.
Our Patient Services Team will always ask what your query is regarding to ensure the appointments available are given for appropriate reasons and with the most appropriate clinician. Our highly trained team comprises of the following clinicians, in addition to our GPs; Practice Nurse, Diabetes/COPD Nurse, Physiotherapist, Clinical Pharmacist & Healthcare Assistant.
If there are no further appointments available, you will be asked to complete an online ‘Klinik’ consultation, this is an online form to detail your symptoms or query, it is sent directly to us and our team will respond within 2-5 working days, although usually same day. Often, this is the most efficient way to get through to the practice and means patients without the technical ability to complete this can still access us through our phone lines.
It is important you cancel any appointment you can no longer attend, you can do this via phone, replying to a text reminder or send us an email to frimleyicb.wellington.practice@nhs.net
Online Access
You will automatically be given online access, this will be emailed to you to the email address provided to us. If you did not provide us with an email address, please contact us to request these details.
The access level for all patients is limited, you will be required to show photo ID to complete the registration for full access. This is detailed in the registration pack you completed & returned to us. Please email us frimleyicb.wellington.practice@nhs.net if you have any queries regarding this.
Annual Reviews for Long Term Conditions
All patients with long term conditions e.g COPD or Diabetes will be invited for an annual review, in addition to any routine appointments you may require throughout the year. The invitation will be sent around the time of your month of birth and we will try and see you in as few appointments as possible for your convenience.
Non NHS Examinations
The NHS does not cover non-medical services such as insurance medicals, medicals for
HGV licenses, fitness to undertake sports or diets etc. We are happy to provide these
services – there is, however, a fee payable in accordance with British Medical Association
guidelines. A list of fees can be here.
If you require something that’s not listed, please check with us.
Vaccinations
We offer regular clinics for all childhood immunisations, and we coincide the 8 week baby vaccines with the 6-8 week postnatal mum check with a GP.

Home Visits
If a home visit is requested, a paramedic practitioner m ay be sent in place of a GP. They liaise fully with the practice and this enables us to give you the best, most efficient care possible.
